About your Union Plus Endorsed Group Accidental Death and Dismemberment Insurance Plan:

Thanks to your union membership with the Union Plus, you are eligible to enroll for up to $200,000 of AD&D Insurance coverage to help your loved ones if you're ever in a covered accident. This coverage pays up to $200,000 REGARDLESS of benefits paid by any other insurance you might have in the unfortunate event of your death. This benefit could help with the cost of a college education, pay off the mortgage or help with living expenses.

Pays Dismemberment Benefits for Certain Accidental Injuries: Your Union Plus sponsored Accident Plan pays benefits for certain covered accidental Injuries, including Loss of vision, hearing, or speech, or the Loss of a hand, foot, thumb, or index finger. The percentage below represents the percentage of the principal sum.

WHAT YOU NEED TO KNOW ABOUT YOUR ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE PLAN COVERAGE

Effective Date:

Your Accidental Death and Dismemberment Insurance coverage starts the first day of the month after your enrollment form and first premium payment are received by the Administrator.

Eligibility:

You are guaranteed acceptance as long as you are an active member, associate member, former member, employee, or retiree of an Adopting Union of the Policyholder, and you reside in the United States. You can keep your coverage as long as the Master Policy stays in force, the premiums are paid when due, and you remain a union member in good standing. Your spouse/domestic partner is eligible if he or she is a U.S. resident at least 18 years old and not legally separated or divorced from you. Your children are eligible as long as they are under age 19, or under age 25 and a full-time student and are not married. Benefits reduce by 50% at age 70 and over.

TERMINATION:

Your coverage terminates on the earlier of: a) the date the Policy is terminated; b) the date You request Your benefits be terminated; c) the date You cease to be a resident of the United States or d) the Premium Due Date on which You fail to pay any required premium, subject to the Individual Grace Period Provision. Coverage for your Eligible Dependents will terminate on the Premium Due Date on or next following the date you cease to be insured or they no longer meet the definition of an Eligible Dependent under the Policy.

EXCLUSIONS:

The policy does not cover any loss caused or contributed by:

Intentionally self-inflicted Injury, suicide or attempted suicide, whether sane or insane; war or act of war, whether declared or not; Injury sustained while On any aircraft except a Civil or Public Aircraft, or Military Transport Aircraft; Injury sustained while On any aircraft: a) as a pilot, flight instructor or examiner-except while performing the duties of a pilot or crew member of a commercial aircraft; b) if it is owned, operated or leased by or on behalf of the Policyholder, or any employer or organization whose eligible persons are covered under the policy; Injury sustained while taking drugs, including but not limited to sedatives, narcotics, barbiturates, amphetamines, or hallucinogens, unless as prescribed by or administered by a Physician; Injury sustained while committing or attempting to commit a felony; or Injury sustained while Intoxicated. Intoxicated means: 1) the blood alcohol content; 2) the results of other means of testing blood alcohol level; or 3) the results of other means of testing other substances; that meet or exceed the legal presumption of intoxication, or under the influence, under the law of the state where the accident occurred.

Injury means bodily injury resulting directly from an accident and independently of all other causes which occurs while the Covered Person is covered under the policy. Loss resulting from: a) sickness or disease, except a pus-forming infection which occurs through an accidental wound; or b) medical or surgical treatment of a sickness or disease; is not considered as resulting from Injury.

Loss means with regard to:

  1. hands and feet, actual severance through or above wrist or ankle joints;
  2. sight, speech or hearing, entire and irrecoverable loss thereof;
  3. thumb and index finger, actual severance through or above the metacarpophalangeal joints.
